Coder I

Summary:

Reports to the Ancillary Coding Manager. Reviews all pertinent documentation, including but not limited to physician documentation, nursing documentation, laboratory and radiology reports, treatment records, and other documentation within the electronic medical record. Selects and sequences appropriate diagnostic codes using ICD-9 CM and/or ICD-10 CM codes: Ancillary Coding – ICD-9/ICD-10 diagnostic and Ancillary Coding – ICD-9/ICD-10 diagnostic coders.

Educational Background:

  • High school diploma or GED required
  • Registered Health Information Technician preferred
  • Bachelor’s degree or Registered Health Information Administrator preferred

Experience Preferred:

  • One year of experience as a coder

Coder II

Summary:

  • Bachelor’s degree or Registered Health Information Administrator preferred

  • Two to three years’ experience

HIM Technician

Summary:

Under general direction, accesses, inputs and retrieves information and problem solves using various computer systems. Acts as liaison for physician offices. Responsible for verification of completed dictation into system. Inputs and retrieves information and problem solves using various computer systems. Acts as liaison for physician offices. Responsible for verification of completed dictation into system. Responsible for Release of Information (ROI) and Birth Registry (BR). In ROI, responsible for providing superior customer service while reviewing and processing information requests from patients, physicians and other medical professionals, insurance companies and others. In BR, responsible for assisting mothers to complete required documentation for the State Department of Vital Records. Maintains confidentiality of medical records in accordance with HIPAA regulations.

Educational Background:

  • Associate’s degree or equivalent education and experience preferred
  • Attain state certification for the department of vital records within 6 months required

Experience Preferred:

  • Health Information Management (HIM) computer software
  • Two years of related records management experience
  • One year of customer service experience, preferably in an acute care setting

HIM Analyst

Summary:

Responsible for maintaining accuracy of electronic medical records with minimal deficiencies and errors. Analyzes and reviews records for required elements and assigns deficient records to appropriate provider for completion. Assures complete and accurate maintenance of key patient data in the EHR information systems. Understands how the various registration and clinical systems interface to trouble-shoot and determine the appropriate course of action when errors are identified. Exercises a high degree of responsibility, problem-solving ability, analytical skills, initiative, and good judgment in resolving duplicate patient records and missing or erroneous data in patient records, including both the EHR and the paper record. May evaluate quality and productivity metrics. May act as liaison to providers to facilitate completion of records in a timely manner.

Educational Background:

  • Associate’s degree or equivalent combination of relevant education and experience required

Experience:

  • Two years of HIM record system evaluation experience, preferably in an acute care setting
  • RHIT preferred

HIM IS/IT Data Analyst

Summary:

Maintains accountability for managing software/data needs and technical services for area of accountability, in collaboration with HIM and Technology and Information Solutions (TIS) staff. Performs the initial analysis of data, develops customized reports to facilitate meaningful interpretation of the data, and provides education support to users.

Educational Background:

  • Bachelor’s degree or equivalent combination of education and experience required

Experience Required:

  • Two years of related and relevant experience
  • Experience with multiple software packages

HIM Supervisor

Summary:

Supervises and oversees the daily activities of the Health Information Management department to ensure the delivery of quality services. Coordinates and monitors health information management programs, policies and procedures, and assumes the responsibility for ensuring that all programs are administered according to company standards and regulatory compliance.

Educational Background:

  • Health Information Management required

HIM Manager

Summary:

Responsible for day-to-day operations of image processing, internal/external release of information, birth certificates, scanning into EMR, physician online record completion, analysis, suspension process, and research projects. Plans, directs, and controls activities and staff within assigned areas of responsibility. Duties and Responsibilities include the following: Organizes, directs, and coordinates the activities of the medical record specialists in image processing and chart completion as well as release of information and birth certificate completion. Facilitates information requests involving research projects. Oversees the quantity and quality of work performed, develops and monitors standards for the quantity and quality of work. Designs methods to improve efficiency and quality. Reports results as directed. Interviews, hires, evaluates, trains, and provides corrective action for staff as necessary. Coordinates 2nd and 3rd shift department activities. Develops and implements policies and procedures for given areas of responsibility and assists in development of departmental policies/procedures. Participates in departmental planning including budgeting for given area of responsibility. Establishes sign-ons and provides training to new users. Serves on or chairs various hospital committees, as required, for effective information management. Acts as liaison with hospital personnel, administration, physicians, governmental agencies/officials, judicial officials and contractors. This position is responsible to assist in development of annual operations budget of approximately $3+ million. This position is also responsible for the day-to-day management of the budget. Assists department in meeting established medical record account receivables targets.

Educational Background:

  • Bachelor’s degree, health information management or associate’s degree in Health Information Management or bachelor’s degree in other field with four years’ experience in medical records supervision
  • Registered Health Information Technician (RHIT) required
  • Registered Health Information Administrator (RHIA) preferred

Experience Preferred:

  • Three to five years’ experience in a health information management/medical records administrative position
